It’s a cumulative effect, not so much the acute dosage (aside from getting to the cumulative total faster).

It worked for me many years ago. I had terrible body and back acne, 20mgs for 5ish months pretty much fixed it entirely.

If your goal is to get rid of the acne in 6 months, yeah, 20mg is perfect.

If you want to get rid of it ASAP, then 40mg/day for the first month and then 20mg/day for the next 4-6 would be great.

Either way, Accutane is a godsend IMO. Besides the obvious sides and other harm it does on your body, after a few weeks, acne will disappear and you will feel great.
